JOB TITLE: Mental Health Care Coordinator

REPORTS TO: Program Manager and Medical Director

DEPARTMENT: John Bell Health Center

LOCATION: Broad Street Ministry

 

Philadelphia FIGHT is seeking an experienced, driven, and highly motivated Licensed Clinical Social

Worker (LCSW) to join FIGHT's satellite integrated primary care clinic team at Broad Street Ministry. This

is a new role that will work to integrate behavioral health care for FIGHT patients, but also for the guests

of Broad Street Ministry, a homeless hospitality service center for those struggling with housing

insecurity. The LCSW will by provide mental health care to the guests of Broad Street Ministry.

Independently the LCSW will work to coordinate across systems citywide to bring mental health care

access to the guests of Broad Street Ministry by working with Community Behavioral Health (CBH), Case

Management teams, and medical teams.

 

The LCSW will be trauma informed, competent in motivational interviewing, and also able to use Cognitive Behavioral

Therapy to identify the individual's goals and values to ensure the guests of BSM are treated with care and respect. Primary

responsibilities include:providing brief on demand mental health support to the guests of Broad Street Ministry,

coordinating mental health care internally and externally to the guests of Broad Street Ministry, and working across systems

city-wide to improve access to mental health care.

 

In 1990, Philadelphia FIGHT began as a clinical research organization with the goal of finding new

treatments and a cure for HIV/AIDS. In the late nineties we opened an HIV Primary Care Clinic (the

Jonathan Lax Treatment Center) and created an HIV education and HIV prevention team. FIGHT

expanded at this time to also serve youth and ex-offenders living with HIV or at risk for HIV. In 2013,

FIGHT became a Federally Qualified Health Center (FQHC). Currently we serve over 6000 patients at five

clinics and two satellite sites including the one at Broad Street Ministry. Overall FIGHT has evolved to

become a comprehensive health services organization serving those challenged by HIV, Hepatitis C, a

history of incarceration, active substance abuse, or a history of drug use. Our goal is to provide an

outstanding standard of care to everyone who comes through our doors.

 

Broad Street Ministry, the location of our BSM Clinic, was established in 2005 when a team of

visionaries, led by Convening Minister and Founder, Rev. Bill Golderer, opened Broad Street Ministry in

the historic Chambers-Wylie Presbyterian Church in order to create a space that would welcome people

from all walks of life. Over time the mission evolved to include serving Philadelphians living in deep

poverty who are experiencing homelessness or housing insecurity. Today, the Hospitality Collaborative is

the flagship program offering five community meals and a wide array of services (mail service, personal

care, mending, clothing closet, therapeutic arts, and counseling) to over 7,000 unique guests per year.

 

"Radical Hospitality," Collaboration, and Civic Engagement permeate through all of BSM's programs and activities, creating a

rich foundation for social impact for its guests. The Mental Health Care Coordinator will work one-on-one with the guests of

Broad Street Ministry, providing mental health support. They will have the power to collaborate with outside providers and

systems to ensure the guests' needs are met with dignity and respect. This position will work onsite at Broad Street Ministry,

Monday through Friday (9-5, opportunity for flexibility in the winter) but will also have the ability to work in our other

clinics as needed. All of our locations are accessible to SEPTA Buses and Subways, Regional Rail, and PATCO. We offer free

continuing education credits during AIDS Education Month. FIGHT is an FQHC and this position would be eligible to apply

for the National Health Service Corps Loan Repayment Program.

 

This is a great opportunity for a senior level LCSW that enjoys quick "therapy," relishes case management, and wants to work

across systems for the benefit of the guest at Broad Street Ministry but also the opportunity to create city-wide system

change.

REQUIREMENTS: Must have an LCSW with 3-5 years of experience

SALARY: $70,000
