Full-Time, 40 hours per week; Exempt

 

Philadelphia FIGHT is an AIDS Service Organization that recently became a Federally Qualified Health Center (FQHC). We have been providing primary care for people living with HIV/AIDS and those at very high risk since 1990. Our program is a team-centered, multi-disciplinary approach that in addition to clinical care, offers access to behavioral health and substance abuse services, a wide range of educational programs, HIV counseling and testing, and youth programs. The Jonathan Lax Treatment Center, our first Health Center, provides culturally competent, non-judgmental, high quality health care to patients living with HIV.  In addition to health care, the Jonathan Lax Treatment Center connects patients with social services, resources, and insurance benefits. 

 

The ideal candidate will take a trauma-informed and harm-reduction approach to health care.

Job Responsibilities

 

Clinical (0.50 FTE)

 

  • Triage sick/emergency walk-ins and sick calls, new patients (take full medical history and determine urgency of illness)
  • Communicate normal and abnormal labs to patients
  • Assist with one-on-one management and teaching of complicated patients with multiple diagnoses, complicated medical regimens, and/or complex social needs
  • Work in collaboration with the PrEP (Pre-Exposure Prophylaxis for HIV) team, to provide HIV prevention to at-risk patients and the Hepatitis C team to provide access to Hepatitis C evaluation and treatment for infected patients.
  • Identify and advocate for patients needing urgent or ongoing behavioral health services and/or substance abuse treatment via collaboration with the primary provider and/or onsite Behavioral Health Consultant.
  • Administer therapeutic injections and vaccinations and perform phlebotomy when needed
  • Perform clinical tasks: i.e. EKG, oxygen set-up, blood glucose monitoring, urine dipsticks and pregnancy testing
  • Perform CLIA related tasks such as running of controls
  • Work as a team with medical receptionists, health information specialists, medical assistants, benefits coordinators, and behavioral health specialists
  • Demonstrate commitment to clinical care and confidentiality
  •  

Administrative (0.5 FTE)

 

  • Supervise Care Coordination Nurse, Community Health Worker and Patient Care Concierge.
  • Assist providers with obtaining urgent reports from diagnostic exams, specialist consults, and hospitalizations
  • In conjunction with an MA, assist providers and nurse manager, in the scheduling of imaging studies and procedures, particularly those requiring prior authorization
  • Assist providers in making specialist appointments for patients who need additional assistance
  • Assist providers with prior authorizations for medications including specialty medications such as Hepatitis C treatments and Vivitrol (an injectable opiate blocking agent)
  • Organize daily clinical huddle with providers, nurses and other support staff
  • Track high risk and complex patients using excel spreadsheets (i.e., patients on Coumadin or Vivitrol etc)
  • Order and coordinate delivery for durable medical equipment
  • Report and assist in communicating appropriate information on reportable diseases to oversight bodies, e.g., Public Health Department, AIDS Activities Coordination Office
  • Coordinate ordering of clinical supplies, including monitoring for expiration of supplies and maintenance of clinic medical equipment (e.g., medical supplies and prescriptions)
  • Participate in monthly Continuous Quality Improvement committee
  • Interact closely with the other clinicians in the practice, Practice Coordinator, Administrative Director and Medical Director, by participating in regular meetings for purposes of addressing management issues and quality assurance.
  • Assist and support outreach and recruitment initiatives, in addition to educational programs as necessary.
  • Alongside other Lax staff, participate in regular cultural competency trainings to best serve our diverse patient population.
  • Demonstrate commitment to clinical care and confidentiality
  • Exercise good judgment in bringing any issues necessary to the attention of the Medical Director and/or the Administrative Director of the Jonathan Lax Treatment Center.
  • Work as a team with medical receptionists, health information specialists, medical assistants, benefits coordinators, behavioral health specialists, community health workers, practice coordinator, and medical providers
  • Participate in monthly Continuous Quality Improvement committee

 

Coordination of Care Provide consultation and coordinate care of patients with clinic staff. 

 

Charting Complete all charting and documentation as needed and required by contracted funding sources, including assessments, progress notes, and administrative documentation.

 

Billing- Complete all billing documentation as needed and required by contracted funding sources, including billing codes and required documentation.

 

Supervision Attend all Lax team meetings, attend FIGHT case conference weekly with all clinicians.

Qualifications

Education: Bachelor's Degree

Certification:

  1. Registered Nurse
  2. Within 30 days of hire, complete training in Blood borne pathogens, Post exposure prophylaxis, and HIPAA regulations.
    1. Prior to shipping blood specimens, complete IATA training for shipping of hazardous substances, if applicable
  3. General knowledge base in science, medicine, and the running of a medical clinic.       Demonstrate the ability to understand the overall purpose of clinical care and importance of support staff.
  4. Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health counselors. Must be able to demonstrate judgment, patience, integrity, organization, and prioritization in multi-task environment. Needs to have attention to detail, dependability and flexibility.

Work Environment:

  • Generally works in a clinical office environment but may occasionally be required to perform job duties outside of the typical clinical setting.

Note: Initial probationary period is three months. Benefits including health benefits as per employee handbook begin on the first day of work.
