HIV Medical Case Manager

Hours: M-F 9:00 to 5:00

Department: Jonathan Lax Treatment Center Location: 1233 Locust St., 5th floor

Reports to: Medical Case Management Supervisor, Director of Medical Case Management Full-Time, 40 hours per week, Exempt

 

Philadelphia FIGHT Community Health Centers (FIGHT) is a comprehensive health services organization providing primary care, consumer education, research, and advocacy for people living with HIV/AIDS and those at high risk. The HIV Medical Case Manager (MCM) will work in an interdisciplinary team setting with medical and behavioral health providers to provide medical case management to adults living with HIV receiving medical care at Philadelphia FIGHT.

 

Tasks and Responsibilities

  • Maintain a caseload of clients living with HIV/AIDS.
  • Complete a biopsychosocial assessment and service care goals with eac client at the time of intake. Monitor and update service care goals quarterly and assessments yearly in coordination with interdisciplinary
  • Maintain contact with clients based on program need, client need, and service care
    • Documentation of adherence to medical appointments and medical care plan, including HIV medication
  • Provide trauma-informed therapeutic support in collaboration with interdisciplinary team, including crisis
  • Identify appropriate resources and external support systems to address client needs including advocating and negotiating linkages to
    • Available to provide home visits and accompany clients to medical appointments and outside treatment facilities as
  • Maintain accurate and timely documentation of all client contacts (using A.P. format), forms, and applications in EHR software.
    • Participate in on-call rotation during regular office

 

Confidentiality and Documentation Standards

The MCM is responsible for maintaining all documentation through service logs and the entry of notes related to the delivery of case management services to the client in a client record, consistent with requirements of the AIDS Activities Coordinating Office (AACO), the Ryan White Modernization Act and Philadelphia FIGHT Community Health Centers. This information is confidential and must be maintained according to applicable law and regulation, specifically including the Health Insurance Portability and Accountability Act (HIPAA) and the Confidentiality of HIV Related Information Act (Act 148). As such these records must be kept in a locked file cabinet or on a secure network provided by Philadelphia FIGHT. At no time is any client specific information to be removed from FIGHT offices, including electronic information which may not be copied onto any external drives and removed.

The MCM is responsible for maintaining all records related to statistical reports required by the agency and the funders to evaluate service performance, as well as records utilized for its continuous quality improvement projects.

 

Qualifications

  1. Master's degree in Social Work (or related field such as public health, psychology, behavioral health, etc.) is required.
  2. Relevant license (e.g. LSW, LPC) or be making substantial progress toward obtaining one.
  3. Knowledge and familiarity with the local social service safety net.
  4. Experience working with people living with HIV/AIDS.
  5. Ability and desire to work productively with highly stigmatized and diverse populations using a strength-based framework.
  6. Excellent interpersonal skills and ability to work collaboratively with clinicians and patients.
  7. MSW and two years post-graduate work experience is preferred.

 

Accountability

  • Initial probationary period is three months.
  • The MCM reports to the Medical Case Management Supervisor and Director of Medical Case Management including timesheets, vacation requests, and weekly supervision.
  • Twenty hours of professional education and training will be required on an annual basis as described by AACO. The AACO core training series must be completed within 12 months of hire.

 

Physical Demands

  • Ability to continuously stand, walk or sit.
  • Ability to work at a computer screen for long periods of time.
  • Ability to bend, climb stairs frequently.
  • Ability to lift up or push to 10-20 pounds infrequently.
  • Ability to squat occasionally.

 

Work Environment

  • Generally works in a clinical office environment but may occasionally be required to perform job duties outside of the typical clinical setting.
