Resource Navigator Job Description


Summary

The Resource Navigator position is responsible for raising awareness of affordable insurance options and providing eligibility and enrollment assistance to uninsured patients of Philadelphia FIGHT's Jonathan Lax Treatment Center, John Bell Health Clinic, YHEP/Peds Health Clinic and Dental Clinic. This includes enrolling patients in PA public benefits, including Medicaid, MAWD, Emergency Medical Assistance, CHIP, WIC, Medicare Extra Help/Medicare Savings Program, SNAP, LIHEAP, CAP/CRP. This position includes assessing patient's eligibility for Philadelphia FIGHT's sliding fee scale. This position is in collaboration with the Office of Community Empowerment and Opportunity, BenePhilly and the William Penn Foundation.


Essential Duties:

  • Assess uninsured patients' potential eligibility for PA Medicaid, CHIP and/or affordable insurance coverage. Complete applications via COMPASS. Support patient applications by following up and advocating with the CAO.
  • Assess patients for Philadelphia FIGHT's sliding fee scale and review Philadelphia FIGHT's policy regarding sliding fee scale.
  • Collect income verification for all patients. Document and upload verifications in EMR.
  • Follow up and track verification documents for public benefit enrollments and sliding fee scale applications.
  • Assess all patient's potential eligibility for all other benefits offered through the BenePhilly and the William Penn Foundation and offer assistance in obtaining these benefits.
  • Assist patients receiving an insurance plan for their specific needs and to receive optimal coverage.
  • Maintain accurate and timely documentation of all patient's contacts (using D.A.P. format), forms, and applications in EHR software.
  • Maintain communication with clinical administrative and case management staff to follow up on the status of patient applications.
  • Complete and comply with all BenePhilly and William Penn Foundation meeting, reporting, and training mandates.
  • Attend department meetings and individual supervision regularly. Comply with all charting requirements.
  • Agree to serve patients' best interests and maintain confidentiality standards in accordance with applicable laws and regulations, specifically including the Health Insurance Portability and Accountability Act (HIPAA) and the Confidentiality of HIV Related Information Act (Act 148).
  • Refer patients to other resources and case management as needed.
  • Contribute as a team member and share in the responsibilities required to maintain operations and serve the mission of Philadelphia FIGHT. This includes, but is not limited to, obtaining training, participating in events, flexibility in scheduling, covering the commitments of coworkers when they are unable, and other duties as assigned.

Qualifications: To perform this job successfully, the individual in this position must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills and ability required.

  • High school diploma or GED equivalent required. Bachelor's degree preferred. Candidates without bachelor's degree but who demonstrate experience and expertise in obtaining health insurance benefits for patients will be strongly considered
  • Familiarity with and understanding of insurance enrollment and Pennsylvania County Assistance benefits applications.
  • Knowledge and familiarity with local social services resources including public benefits.
  • Experience and proficiency with Microsoft Office required. Additional experience with online benefit application databases and Electronic Medical Record programs preferred.
  • Ability and desire to work productively with highly stigmatized and diverse populations using a strength-based framework.
  • Excellent interpersonal skills and ability to work collaboratively with patients and families.
  • Experience working with people living with HIV/AIDS and other chronic health conditions highly desired.
  • Candidates must possess strong communication skills, the ability to multi-task and excellent an attention to detail and follow-up skills.
  • Bilingual (Spanish/English) is desired.

Required Screenings:

  • Must complete Philadelphia FIGHT Credentialing and Privileging to provide direct services with Federally Qualified Health Centers.

Physical and Mental Requirements: Ability to work under pressure and be creative and analytical. Ability to continuously stand, walk or sit. Ability to work at a computer screen for long periods of time.

Salary Range: $46,000 - $53,000. Competitive employee benefits; medical, dental & vision insurance begins on first day of employment.

Work Hours: 40 Hours/Week, Non-exempt

Work Environment: Generally, works in a clinical office environment but may occasionally be required to perform job duties outside of the typical clinical setting. Noise - Moderate noise (example: business office with computers and printers, light traffic).
