LPN Job Description
We are seeking a passionate and motivated LPN to provide clinical and administrative services at Philadelphia FIGHT's John Bell Health Center. This care is offered in accordance with Philadelphia FIGHT's mission to reduce health disparities for vulnerable populations - including, but not limited to, those living with HIV and those at high risk, homeless and transient youth, members of the LGBT community, formerly incarcerated persons, and low-income members of the community.
Philadelphia FIGHT was initially founded in 1990 as a comprehensive AIDS service organization. Today we are a Federally Qualified Health Center (FQHC) with clinical sites that provide state-of-the-art culturally competent primary care, advocacy, social services, consumer education and outreach for patients in need regardless of HIV or insurance status.
Qualifications: To perform this job successfully, the individual in this position must have a HS Diploma/GED or higher and currently hold an active LPN license in the state of PA.
Responsibilities:
Essential job duties for this position include the following items. Other duties must be performed as assigned or required.
Primary Care:
- Taking vital signs
- Recording patient's medical history
- Giving patients injections (vaccines, Apretude, Sublocade)
- Offering basic levels of care such as changing of bandages, cleaning and dressing wounds
- Collecting samples for routine laboratory testing
- Reinforcing teachings done by providers - You will help patients understand what they should do post-treatment or how the family should care for a relative who's sick or injured
- Assisting medical staff with basic medical tasks, such as administering medication
- Update patient records in a confidential manner
- Explain treatments and procedures to patients
- Health Assessments
- Triage patient phone calls and inbox messages on the portal
- Support new office programs, i.e., give testosterone injections, COVID injections etc. to new patients.
- Promote wellness by providing patient education materials and communicating the physician advice/instructions, answering questions that arise
- Participate in population health and community outreach incentives.
- When needed, support medical assistant in administering injections and vaccinations, and performing phlebotomy.
- Comply with requirements of appropriate regulatory agencies (i.e., OSHA, CLIA, PA Department of Health).
- Administer medications including oral, SQ injection, IM injection.
- Comply with all policies and procedures.
- Document all activities/interventions, patient/family responses, medication dispensed/prescribed, etc., in the medical record.
- Update record of medications
- Assists with patient chart documentation
- Support Medical Assistants in perform clinical tasks as needed: i.e., EKG, nebulizer treatments, blood glucose monitoring, urine dipsticks and pregnancy testing, STD testing.
- Work as a collaborative team with the entire JBHC medical staff, including medical providers, behavioral health consultant, therapists, medical assistants, benefits coordinator and practice coordinator.
Other: Initial probationary period is three months.
Work Hours: 40 Hours/Week (M-F 9:00am - 5:00pm); Exempt
Work Environment: Generally, works in an office environment with moderate noise levels.
