PrEP Retention Coordinator Job Description


Philadelphia FIGHT Community Health Center's HIV Pre-Exposure Prophylaxis Program is a nationally recognized program in PrEP delivery and implementation, as well as patient care. We are looking to establish a full-time PrEP Retention Coordinator Positon at the John Bell Health Center to support the clinical team in PrEP care and work collaboratively with the PrEP team at FIGHT in PrEP expansion across the health centers.

This position will be the primary on-site staff person supporting the development of John Bell Health Center's PrEP & primary care initiative. The Pre-Exposure Prophylaxis (PrEP) Retention Coordinator will play a key role in the development and expansion of the PrEP resources including but not limited to: education, recruitment, engagement and retention, patient navigation and adherence counseling program at the John Bell Health Center.


Qualifications: High school diploma required. Bachelor's degree is preferred. Knowledge in science and medicine; knowledge of PrEP and combination HIV prevention and treatment. 1-2 years of experience conducting non-medical assessments and client centered education. Sensitive to cultural, economic, gender, and sexual orientation differences.

Skills:

  • PA HIV Testing Certification is preferred.
  • Proven track record of organization, attention to detail and ability to respond promptly to requests, anticipate agency needs, and maintain efficient clinical operations.
  • Previous experience using an Electronic Medical Record System (EMR) is preferred
  • Strong computer literacy with proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint).
  • Ability to work across teams effectively.

Responsibilities include:

The duties required of the PrEP Retention Coordinator will include, but not be limited to:

  • Conduct non-medical assessments for PrEP services
  • Conduct client-centered PrEP education and adherence counseling services with a focus on client engagement and retention.
  • Provide integrated PrEP counseling to potential and current participants in PrEP program
  • Provide ongoing education about when and how to stop PrEP if desired, when and how to reinitiate PrEP in the future, when to call with questions, and other important questions
  • Support clinical staff at JBHC to refer to the PrEP program
  • Keep a log of PrEP patients
    • Update and maintain a schedule of required lab visits and medication refill schedules
    • Ensure appointments are scheduled at least every 3 months for each PrEP patients
    • Contact patients who miss appointments or do not come in for laboratory testing or refills as part of our on-going lost-to-care initiative
  • Manage patient assistance for medication access for all patients accessing PrEP at JBHC
  • Cultivate and maintain community partnerships through PrEP outreach
  • Work closely with the staff of the Institute for Community Justice to provide PrEP education
  • Develop marketing and social media in conjunction with PrEP Director
  • Act as a Resource Navigator for patients receiving PrEP and refer to appropriate services within JBHC
  • Support PrEP participants who may be newly diagnosed with HIV in linkage to HIV care and treatment
  • Support community outreach initiatives and work with the community engagement coordinator to attend outreach events as needed
  • Attend meetings, case conferences, and supervisions
  • Other duties as assigned

Other: Initial probationary period is three months. Performance of other tasks may be assigned on an as-needed basis.

Work Environment: Generally, works in an office environment.

Salary range: Non-exempt; Competitive salary

Work Hours: 40 Hours/Week (M-F 9:00am - 5:00pm)
